*SPECIAL PROVISIONS REGARDING ROLL-OFFS*
In order to provide excellent service for your company, we ask that you follow the guidelines listed below:
* The empty weight of our truck is 36,800lbs. Our gross weight for hauling is 54,800lbs. If the container scheduled for dump exceeds 18,000lbs, the undersigned will take full responsibility for any fees that J & E Recycling may incur regarding DOT weight restrictions.
* Before scheduling any container activity, always make sure that we have access to the container and the following filling requirements have been met:
- Make sure debris is not protruding over the top of the container. If the container is
overloaded at the time of scheduled dump, then, at Company’s discretion, any excess debris will be unloaded from the container at that time or service will be refused.
- When filling a container with bricks, blocks, concrete, dirt, sand, and any material
that is excessively heavy, only fill the container one-half full. Do not mix with Trash.
- We are prohibited from accepting the following items: Tires, Household garbage, Batteries, Bio-hazardous, Medical waste, Liquids, Chemicals, Sludge, Slurry, Asbestos and Contaminated soils.
There will be an $85.00 trip charge for any service requested that we are unable to perform because of inaccessibility to the container or because of a violation of the above-listed filling requirements.
* There will be an additional $100.00 charge for any stumps and brush placed in the container.
* For any excessive damage occurring to the containers due to over-filling or moving of the container, a fee will be assessed to cover the damage.
* Our containers are rented on a 30-day basis and will be subject to a $5.00 per day rental fee if there is no activity after the first 30-day period.
The individual signing below on behalf of the Customer acknowledges that he or she has read and understands the Special Provisions Regarding Roll-Offs listed above, and that he or she has the authority to sign on behalf of the Customer.
